By scheduling an appointment with Slice Telehealth Services, you acknowledge and agree that:
  • Services are provided via telehealth and may have limitations compared to in-person care.
  • Telehealth is not appropriate for emergencies. Call 9-1-1 if needed.
  • No provider–patient relationship is established until your appointment is completed.
  • You consent to electronic communication, documentation, and treatment.
  • You confirm you are physically located in Illinois at the time of service.

To complete your onboarding and keep your visit moving smoothly, please review our full consent packet and electronically sign your required acknowledgements. These consents are required for care delivery, documentation, and billing compliance, and are typically completed one time per patient and stored on file.
